Pre-payment is required to secure your telehealth appointment. Once payment is completed, your appointment will be confirmed, and you will receive a receipt along with the necessary details. We accept various payment methods for your convenience.
The practice does /does NOT file for insurance reimbursement. All services are paid for by the patient at the time of service. You may pay cash, credit card, HSA card, or Flexible Spending Card. We will provide you with a superbill with all the necessary codes, so that you may file for reimbursement with your insurance company.
As the patient, it is in your best interest to know and understand your insurance plan benefits. It is important that you know your benefits prior to visiting. Regardless of your individual insurance coverage or type, as the person seeking medical treatment, you are ultimately responsible for all charges.
All outstanding balances must be paid in full prior to the next office visit or receiving supplements.

This office requires 24-hour notice if you are unable to keep your new patient or initial visit and 24-hour notice if you cannot keep your follow-up or routine visit. If you miss an appointment or fail to give sufficient notice, you will be charged $75.00 for that missed appointment. This payment is expected before any further treatment is provided, or supplements can be purchased.

Phone calls requiring 10 minutes or more of the provider's time will be charged as a minimum visit ($40/15 minutes).

At ProActive Functional Health, we value your time and strive to provide the best care possible. To ensure that all our patients have access to timely appointments, we have implemented a no-call, no-show policy.

Please be aware that if you do not call to cancel or reschedule and do not show up for your appointment, 100% of the funds for the scheduled service will be retained. This policy helps us maintain the highest level of service for all our patients. We encourage you to contact our office to discuss how we can better accommodate your scheduling needs.

We are committed to providing the highest quality of care and service to our patients. We understand that there may be circumstances where a refund is necessary. Below is our refund policy to ensure clarity and transparency:
Appointment Cancellations and Refunds:
If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Appointments canceled with at least 24 hours' notice are eligible for a full refund or can be rescheduled at no additional charge.
If we need to cancel your appointment for any reason, we will reach out to you to reschedule.However, you will receive a 100% refund should you choose not to continue with the appointment.
If you wish to continue with your visit but need to reschedule, please do so via the patient portal. This allows for efficient management of your appointment and ensures you receive timely care.
Supplements and Lab Orders:
For issues with supplements or lab orders, please refer to the FullScript Support Team, as they are an external company we contract with.
Processing Refunds:
Refunds will be processed using the original payment method within 7-10 business days of approval.
Please be aware that it may take additional time for your bank or credit card provider to reflect the refund in your account.

Please note that we are not contracted with any insurance carriers. All individual health plans vary, and as your insurance benefits are a contract between you and/or your employer and your insurance company, it is your responsibility to know your benefits, eligibility, and liabilities prior to making an appointment.
You will be responsible for paying all charges at the time of service, as we are a debit/credit card-only practice. It may be helpful to contact your insurance customer service department to inquire about reimbursement eligibility for your visit before committing to an appointment. Specifically, ask for coverage information on your benefits and eligibility for naturopathic care. Insurance may cover all or a portion of the office visit and some labs. Unfortunately, Medicare does not cover functional medicine at this time, and we do not accept Medicaid Insurance.
We are not responsible for payment discrepancies that might occur once the reimbursement check is received. It is the patient's responsibility to keep track of their deductible, maximum benefit, or other liabilities specific to their plan's coverage. (Weight loss programs are not covered by insurance).

For uninsured patients, please be aware that we do not offer a sliding fee scale. You will be treated as debit/credit pay at the time of service. All charges must be paid in full. We accept debit and credit card payments only. Please ensure that you are prepared to cover the cost of your treatment, as we do not offer payment plans or defer payments for uninsured individuals.

Telehealth sessions are intended to provide clients with convenient access to healthcare services and to enhance their understanding of their health concerns. However, it is important to note the following:
Nature of Telehealth Services: Sessions are conducted through telehealth, which includes the use of electronic communications to share medical information with a healthcare provider who is not in the same location as the patient. Telehealth services are subject to limitations, including but not limited to the lack of physical examination.
Not a Substitute for In-Person Care: While telehealth can be highly effective, it is not a substitute for in-person visits when such visits are necessary. We encourage clients to continue seeing their primary care provider and specialty providers according to their plan of care.
No Provider-Patient Relationship: Participation in this telehealth session does not establish a provider-patient relationship. The information provided is intended for informational purposes only and should not be considered as a comprehensive healthcare plan.
Confidentiality and Privacy: Efforts are made to ensure the security and confidentiality of client information during telehealth sessions. However, there are potential risks associated with the use of electronic communication, including unauthorized access, technical failures, or other issues beyond our control.
Emergency Situations: Telehealth services are not suitable for emergency situations. In the event of a medical emergency, clients should call 911 or visit the nearest emergency room immediately.
Client Responsibilities: Patients are responsible for providing accurate and complete information about their health history and current symptoms. It is also important for patients to ensure they are in a private location during the telehealth session to maintain their privacy.
Age Restriction: Telehealth services are available to individuals who are 18 years of age or older. By participating in this telehealth session, patients confirm that they meet this age requirement.
By proceeding with a telehealth session, patients acknowledge that they have read and understood this disclaimer and agree to its terms.

Please be advised that ProActive Functional Health does not accept insurance. All services provided are strictly on a debit/credit payment basis at the time of service. It is the patient’s responsibility to manage any potential reimbursement with their insurance carrier.
Due to varying rules and policies among different insurance providers, we strongly recommend that patients consult with their insurance company prior to receiving services to understand their coverage and reimbursement options.
ProActive Functional Health takes no responsibility for any non-reimbursement from insurance carriers. All financial liabilities, including those related to insurance claims, remain the sole responsibility of the patient.

Many supplements, vitamins, medical grade foods, nutritional powders, botanicals, and homeopathic remedies have not been evaluated by the US Food & Drug Administration (FDA) and these products are not intended to diagnose, treat, cure, or prevent any disease.

Telemedicine appointments will be conducted similarly to our in-person visits, so please plan to spend up to an hour on your device. We recognize the importance of your time and strive to maintain efficiency and punctuality with every appointment; however, occasional delays may occur. If your appointment has not started within 15 minutes of the scheduled time, please contact our office by phone or email.
If you are scheduled for a telemedicine visit, our office will send you a text or email with a link to your video appointment in advance. Please click the link 5 minutes before your scheduled start time. Should you encounter any difficulties joining the session, please call our office for assistance with troubleshooting.

Technology Requirements:
Ensure you have a stable internet connection.
Use a device with a camera and microphone, such as a smartphone, tablet, or computer.
Setting Up:
Find a quiet, private space for your appointment to discuss health matters confidentially.
Test your audio and video settings prior to your appointment.
Appointment Access:
You will receive a link via email or text to join the telehealth session. Please log in 5-10 minutes early to troubleshoot any technical issues.
Documentation:
Submit your telehealth consent form along with the consultation and evaluation form prior to your appointment. Ensuring these are submitted in advance will streamline the process and make our time together more productive.
Have any other relevant medical records, medications, or questions ready to discuss with your provider.
Patient Responsibility:
It is your responsibility to meet these criteria to ensure an uninterrupted telehealth experience. Rescheduling due to technical difficulties may result in a delay in care.
Alternative Options:
If you do not have access to the necessary technology or lack coverage, you may still have a visit. You have the option of a phone consultation or video consultation in this case.
